Illinois state law requires that all students entering 9th grade to have a current physical and complete immunization record.
Illinois state law also requires that all students entering 12th grade must show proof of having received two doses of the meningococcal conjugate vaccine before entering the 12th grade.
Contact your child's primary care provider to review your child's immunization record to ensure they are in compliance with these requirements. Proof of immunity is not allowed.
Acceptable documentation of the meningococcal vaccine includes any of the following:
- Signed note from physician that shows the date the vaccine was given
- Printed copy of your child's immunization record
- Copy of child's school physical form that specifies the exact date(s) of the vaccine
Verification of this vaccine must be turned in to the school nurse as soon as possible. Students will not be able to complete the registration process for the 2015-2016 school year and will not be able to attend school without documented proof of the meningococcal vaccine and current physical.
